In a device like a lamp or any appliance with a metal frame, the frame is attached to the ground wire. Usually, the 3rd prong, the grounding prong is used for safety. You can plug the adapter in one way, and then flip it upside-down and reverse the polarity and the adapter works just fine. On the adapter there are two blades and both are small (hot) sized, meaning there is no set polarity orientation on the adapter. If the 3rd slot is on the bottom, then that's left/large/neutral and right/small/hot. Newer outlets have polarized two-slots with the smaller slot being hot, and the larger slot being neutral.
